Erbium:YAG Laser Ablation of Osteoma Cutis: Modifications of the Approach

We read with great interest the article by Hughes1 on the treatment of multiple miliary osteoma cutis with an erbium:YAG laser. This approach is not new, and the article convincingly confirms our earlier treatment results of facial osteoma cutis.2-3
However, in our experience a pretreatment with topical tretinoin/lactic acid was not necessary for good results. Furthermore, the procedure could well be performed using topical prilocaine/lidocaine (EMLA; Astra, Wedel, Germany) cream if the entire face was not treated in 1 session. Because the size and extent of some osteomas in the dermis are unpredictable and cannot always be estimated in advance, tender ablation is necessary to avoid damage to the surrounding tissue. And while bony material can be efficiently ablated with the erbium:YAG laser,4 the absorption characteristics of dermal tissue and bone vary.
