Warty Nodules on an Annular Plaque in a Renal Transplant Recipient—Quiz Case

REPORT OF A CASE

A 50-year-old Chinese man who had undergone renal transplantation 4 years earlier and who was currently taking oral cyclosporine (210 mg/d), azathioprine (50 mg/d), and prednisolone (10 mg/d) presented with a 11/2-year history of a lesion on his left shin. It had started as a small patch and had gradually increased in size. It was neither itchy nor painful. He also noticed 3 asymptomatic nodules that had appeared over the initial patch and had gradually increased in size over the past 8 months.

Physical examination revealed a 7-cm dry, scaly, annular plaque over the left shin. There were 3 warty, hyperkeratotic nodules arising from the scaly plaque (Figure 1). Biopsy specimens were obtained from the edge of the plaque (Figure 2) and from the nodule (Figure 3).
