The lesions shown are from the left (Figure 1) and right (Figure 2) sides of the back of a 61-year-old white man and from the left thigh of a 66-year-old white woman (Figure 3) (size bar, 5 mm). All 3 of these lesions reveal a similar pattern. They have a foggy brown quality punctuated by hypopigmented areas. In contrast to other seborrheic keratosis patterns, the hypopigmentation in these lesions is largely not from milialike cysts but instead appears to be due to the underlying skin structures including follicular structures. This pattern may represent an earlier stage of keratosis formation.
