The lesions shown are from the back of a 12-year-old white boy (Figure 1), the leg of a 21-year-old white woman (Figure 2), and the abdomen of a 31-year-old white woman (Figure 3). The lesions reveal a targetoid appearance with the blue pigmentation being present centrally (best represented by Figures 1 and 2) within a larger lesion demonstrating brown/tan epidermal pigmentation. The etiology of these lesions is unknown but it is possible that an underlying mutation results in a clone of cells that expands preferentially in the epidermis but only focally in the dermis.
