Diagnosis: Histoid leprosy.
MICROSCOPIC FINDINGS AND CLINICAL COURSE
The histologic sections showed a well-circumscribed nodular proliferation of histiocytes in the superficial and deep dermis. A grenz zone was present. The histiocytes were spindled at the periphery of the nodule. They had a moderate amount of eosinophilic cytoplasm and oval nuclei. Occasional cells were foamy, but they did not have the marked cytoplasmic vacuolation that is characteristic of lepra cells. The Fite stain demonstrated numerous intracytoplasmic, long, acid-fast bacilli.
The patient was started on a regimen of dapsone (100 mg/d), rifampin (600 mg/d), and clofazamine (100 mg/d). Two months later, he developed erythema nodosum leprosum, and thalidomide therapy (200 mg/d) was initiated. The erythema nodosum leprosum improved, and the dosage of the thalidomide therapy was tapered to 50 mg/d.
