Diagnosis: Hydroa vacciniforme (HV).
MICROSCOPIC FINDINGS AND CLINICAL COURSE
The histologic sections showed an intraepidermal blister, with confluent necrosis of the epidermis, resulting in eosinophilic reticular degeneration. Regenerating epidermis was seen at the periphery. Dermal papillary edema was evident, and dermal vessels at the base of the necrotic area were filled with red blood cell thrombi. There was also a moderately dense perivascular infiltrate of lymphocytes and histiocytes. The histologic findings were consistent with HV. The results of in situ hybridization for Epstein-Barr virus were negative.
The findings of blood investigations (complete blood cell count and serologic tests for antinuclear antibody), urinalysis, and porphyrin studies were all unremarkable. Phototesting was carried out using UV-A (Mutzhas, Munich, Germany) and UV-B (Dermaray; Tokyo, Japan) light sources. The minimal erythema doses for UV-A (>100 J/cm2) and UV-B (>100 mJ/cm2) were normal for our patient's skin type. He declined to undergo photoprovocation tests.
