My coauthors and I appreciate the positive response to our study on intralesional lipolysis, and we share the opinion of Dr Bechara and colleagues that monotherapy with injections of PTC is unable to completely erase lipomas.1 In some patients, size reduction of grossly 50% of the pretreatment volume offers sufficient relief from the disturbance. The fact that it may be easier for surgeons to remove lipomas after 3 or more intralesional injections of PTCperhaps by means of the "squeeze delivery" techniquesupports the feasibility of this treatment option.2
Still, at this stage, intralesional lipolysis cannot be recommended as a standard procedure. Recurrence rates, recommended dosage, and remaining uncertainties should be carefully studied before this method is used for the reduction of lipomas and for fat reduction for aesthetic purposes. Further controlled clinical trials are necessary to create an evidence-based scientific background for intralesional application of PTC formulations.
