Failure of Etanercept Therapy in Disseminated Granuloma Annulare

With great interest, we read the article by Shupack and Siu1 published in the March 2006 issue of the ARCHIVES. The authors describe a patient with a 20-month history of disseminated granuloma annulare (GA) who responded well to etanercept therapy. Unfortunately we cannot share their positive experience of etanercept treatment for this indication.
Encouraged by a report of successful infliximab treatment in disseminated GA,2 we initiated etanercept therapy in 4 patients with GA unresponsive to prior conventional regimens. The patient characteristics are summarized in the Table. To evaluate the optimal dose of etanercept, both 25- and 50-mg twice-weekly applications were used. None of the patients showed any clinical improvement after 12 weeks of treatment. Additionally, no differences in dermal thickness and density were observed under 20-MHz ultrasound scanning of representative skin lesions before and after treatment.
