Eroded Patch on the Nipple of a 57-Year-Old Woman—Diagnosis

Diagnosis: Erosive adenomatosis of the nipple (EAN).

MICROSCOPIC FINDINGS AND CLINICAL COURSE

Microscopic examination of the diagnostic shave biopsy specimen revealed an endophytic epithelial neoplasm arising from the base of the epidermis. The lobules of epithelial cells invading the dermis variably exhibited either a glandular or solid morphology. Decapitation secretion was present. An immunoperoxidase stain for smooth muscle actin confirmed the presence of an outer myoepithelial cell layer on the proliferating ductal epithelium. The histological and immunohistochemical findings were consistent with a diagnosis of EAN. Complete excision was achieved with Mohs micrographic surgery. The tissue-sparing benefits allowed for immediate aesthetic reconstruction of the nipple.

DISCUSSION

Erosive adenomatosis of the nipple is a rare, benign, adenomatous proliferation of the lactiferous ducts of the nipple. Since the initial report by Jones1 on "florid papillomatosis of the nipple ducts" in 1955, this entity has also been termed papillary adenoma, superficial papillary adenomatosis, nipple duct adenoma, and subareolar papillomatosis of the nipple.
