Classic Kaposi sarcoma (KS), a rare vascular tumor, may be treated with photodynamic therapy (PDT). However, for thick lesions the role of topical PDT is limited owing to reduced penetration.

Report of a Case

A 70-year-old man presented with a 6-month history of skin lesions on the foot. He had no significant medical history and was heterosexual. Physical examination revealed multiple nontender, purple papules on the left foot (Figure 1A). A skin biopsy specimen showed abnormal vascular structures and perivascular proliferation of spindle cells in the dermis (Figure 2A). The cells stained positively for CD31 and human herpesvirus 8 (HHV-8) (Figure 2C). Results of blood tests for HIV were negative.
