Tender Papule of the Nail Fold—Diagnosis

Diagnosis: Superficial acral fibromyxoma (SAF).

MICROSCOPIC FINDINGS

Microscopic examination shows a population of plump, spindled cells in the dermis. The cells are arranged in loose fascicles within a myxoid stroma. Other areas of the specimen show thickened collagen bundles (Figure 2 and Figure 3). Mast cells are prominent throughout the specimen. The neoplastic cells are CD34+ positive and negative for S-100 protein.

DISCUSSION

Superficial acral fibromyxoma is an entity described by Fetsch et al,1 who published a series of 37 tumors. Several additional case reports and a more recent series of 32 tumors have also been described in the literature.2-6 Usually SAFs are slow-growing, painless tumors that range in size from 0.6 to 5.0 cm (mean size, 1.75 cm).
