Immediate Diagnosis of Cryptococcus Fungal Infection Using Touch Preparation Analysis

We report the diagnosis of Cryptococcus infection by rapid touch preparation analysis (hereinafter, "touch prep") of a skin punch biopsy specimen in a patient undergoing liver transplantation. The diagnosis was subsequently confirmed by histologic findings.
Report of a Case
A 52-year-old man presented with the acute onset of multiple red, tender skin nodules, ranging in size from 2 to 3 cm in diameter, on the posterior aspect of his left arm and right leg. He had a history of end-stage liver disease secondary to hemachromatosis and had undergone orthotopic liver transplantation 4 weeks earlier. Three punch biopsy specimens were obtained from nodules on both arms and the right thigh for routine histologic evaluation and for tissue culture.
