To the Editor.—

In their article "Papular Dermatitis Herpetiformis in Childhood" (ARCH DERM100:286-290, 1969) Drs. Ackerman and Tolman make a case for their position that the diagnostic criteria for dermatitis herpetiformis (DH) in children should be the same as the criteria used in diagnosing DH in an adult. They would diagnose as bullous pemphigoid those cases reported as DH in a child which were predominantly bullous, nonpruritic, and unresponsive to sulfones. We have had experience with both of these entities in children and would question the finality of their conclusion. While some cases of DH in childhood do fit the criteria of adult DH, we have seen other cases in children that were predominantly bullous and nonpruritic and yet were very responsive to one of the sulfones.
