Artifact or Vasculitis. Presented by RICHARD C. GIBBS, MD.

A 47-year-old woman first noted unusual discoloration of the skin of the right foot and ankle about 16 years ago. Initially there was spontaneous clearing but each year for the past six years she has had several episodes of ankle cellulitis. Examination for possible large vessel disease was normal. A pruritic eruption consisting of erythematous scaly papules appeared on her arms 18 months ago and was diagnosed lichen planus. Her arms have been clear for the past several months.

There is an erythematous, excoriated, and hemorrhagic area measuring about 10 cm over the medial and lateral aspect of both ankles (Fig 1).

Histopathologic examination one year ago of skin from the forearm revealed a subacute dermatitis. Two years ago a biopsied specimen from an ankle showed scattered collections of small round cells . . . [Full Text PDF of this Article]
