Atrophoderma (Type Unknown). Presented by Arnold W. Gurevitch, MD, Dore J. Gilbert, MD.
This 20-year-old woman has had an asymptomatic eruption on her legs for 3
years, with recent spread to her trunk and right upper arm. She also complains of tender, red areas on her buttocks that she believes are secondary to being bathed in chlorine bleach as a child.
Various topical creams have produced no beneficial results.
Multiple macular, erythematous lesions are seen on both thighs, within which are regions of circumscribed atrophy. The right upper arm, as well as the trunk, reveals small pink papules. The buttocks show extensive bilateral and symmetrical atrophic, tender, and very erythematous areas, with marked wrinkling of the skin.
Four punch biopsy specimens were taken from normal skin, a small erythematous papule, an atrophic macule, and from the atrophic buttock lesion. They were stained with hematoxylin-eosin and for elastic tissue.
