Mycobacterium marinum Granuloma. Presented by PAUL J. CATINELLA, MD

This 24-year-old man's current problem began in March 1976 with the onset of an asymptomatic, erythematous, scaling nodule, localized to the right third finger, which has slowly increased in size over the past year. He has been employed by a local retail store that deals in tropical fish and he has maintained and cleaned several different kinds of aquariums during the past year.

Examination showed a 1.5 x 2-cm irregular, erythematous, firm, raised nodular lesion over the knuckle of the third right metacarpophalangeal joint.

Biopsy disclosed a granulomatous response of undetermined etiology following multiple histochemical studies. Mycobac terium marinum, sensitive only to cycloserine, grew in cultures of the biopsy specimen. Therapy has not been administered because of the unusual resistance of the . . .
