To the Editor.—

I read with interest the article in the April issue of the ARCHIVES on successful treatment of erythrokeratodermia variabilis (EKV) with isotretinoin.¹ In 1981, we reported a case of EKV that was treated successfully with etretinate.² In our case, however, we could observe no abnormalities in the quantity of keratinosomes (Odland bodies) before treatment. Our results for tritiated thymidine autoradiography and electron microscopy were indicative of a proliferation hyperkeratosis together with retention hyperkeratosis. Whether the controversial results in our patient and Rappaport's patient are caused by different or insufficient techniques in investigation, or whether several morphological subtypes of EKV exist, remains unclear. . . .
