REPORT OF A CASE
A 19-year-old man presented with a four-week history of asymptomatic papules on the shaft of the penis. No similar lesions were present elsewhere. Although he had been sexually active, there was no correlation with sexual intercourse. No other family member had a similar problem.
On physical examination, there were multiple, round to oval, flesh-colored, slightly yellow soft dermal papules, approximately 4 to 7 mm in greatest dimension, on dorsolateral surface of shaft of penis (Figs 1 and 2).
Examination of a biopsy specimen of a papule reveals a dermal proliferation of epithelial nests, most of which contain a central ductal space filled with amorphous pink granular secretions. Solid-appearing dermal epithelial cords and nests are also seen. In the intervening stroma, there is a slight increase in both fibroblasts and dense collagen in the intervening stroma (Fig 3). Examination with higher magnification better demonstrates both solid epithelial islands as well as those with ductal structures containing secretions (Fig 4).
