REPORT OF A CASE

A 4-year-old black girl presented with a 9-day history of fever and rash. A fine, papular, pruritic rash developed on her face resembling "heat rash," and there was crusting of the scalp. Three days later, the rash became thick and scaly, spreading to her trunk and arms, and a fever developed.

On admission, she was febrile. Diffuse, thick scale covered the scalp with no areas of alopecia. The face and ears showed extensive scaly and crusted dirty-brown papules (Figure 1). On the neck, trunk, and extremities, scaly papules and annular plaques were present (Figure 2).

Laboratory examination revealed a normal complete blood cell count and differential and serum chemistry values. Cultures of the blood and urine were sterile at 72 hours, and multiple potassium hydroxide (KOH) preparations of the hair, scalp, and skin were negative for hyphae.
