Cytomegalovirus Inclusion Disease Presenting as a Cutaneous Ulcer in AIDS

REPORT OF A CASE
A 27-year-old woman with acquired immunodeficiency syndrome (AIDS) presented with a 2-month history of a 1.5-cm ulcerated erythematous plaque on her right thigh (Figure 1). A similar 8-mm lesion was present on the dorsal right hand. The patient had a 1-month history of cough productive of white sputum, anorexia, vomiting, and a 9-kg weight loss. She complained of decreased vision in her left eye of several weeks' duration. Laboratory evaluation demonstrated the following: leukocytes, 4.6x109/L; alkaline phosphatase, 298 U/L; lactic dehydrogenase, 695 U/L; aspartate aminotransferase, 216 U/L; alanine amino- transferase, 277 U/L; and serology for hepatitis A, B, and C, negative. Chest roentgenogram demonstrated right paratracheal prominence with a hazy perihilar infiltrate on the right. Sputum cultures were negative and the patient was anergic to skin testing.
