REPORT OF A CASE

A 34-year-old Japanese-American man presented with a 20x10-cm plaque on his chest. It appeared 4 months previously as a group of papules, which expanded centrifugally. The patient complained of moderate pruritus. The lesions clinically waxed and waned. His history was significant for a similar lesion on the right arm that improved with use of a topical cream. Physical examination revealed a 20x10-cm centrally hyperpigmented plaque located on the upper anterior central part of the chest (Figure 1). Along the lesion's periphery were numerous erythematous papules and pustules (Figure 2). The remainder of his examination was unremarkable, and the complete blood cell count was normal.

The patient was treated with desoximetasone cream for 3 weeks. On follow-up, the lesion had progressed with a central clear area and peripheral erythematous papules. A punch biopsy specimen was interpreted as "acute necrotizing folliculitis, possibly of bacterial origin."
