Itching about the anus is a symptom secondary to many disorders.¹ It may be caused by local conditions, such as anal fissures, fistulas, infected crypts, condylomas, the presence of pinworms or pediculi and hemorrhoids. Reflexly, diseases of the genito-urinary and of the gastrointestinal tract may give rise to anal itching, or the condition may be due to constitutional diseases, such as allergy, diabetes and gout. However, in most cases, anal itching occurs without these conditions, and there is a distinct clinical entity of primary pruritus ani. This essential pruritus ani has been ascribed by various investigators to bacterial or fungous infection of the skin, to irritation of mucus in the sinuses within the perianal skin and to "neurosis." Whatever the cause may be, there is no question that in pruritus ani of long standing the skin is infected with bacteria, and occasionally mycelial segments are found.
